Long Term Disability (LTD) Coverage

Long Term Disability (LTD) provides income to employees who are
disabled for an extended period of time. The LTD plan from Lincoln
is designed to help protect your income in the event that you are
unable to work due to a covered disability.

Your coverage effective date or any increase in coverage may be
delayed if you are disabled on the date coverage is scheduled to
take effect. Review the Summary Plan Description for details.

LTD Benefits

Monthly Benefit 60% of Pre-Disability earnings

Elimination Period 180 Days

Minimum/Maximum $100 or 10% (whichever is greater) / $10,000 per month
Monthly Benefit

The later of Your Normal Retirement Age as defined by Social Security or the
period shown below:
