HFACS TIERS


     HFACS consists of four broad categories of factors known to
     contribute to errors and accidents. These include:

        Unsafe Acts
        Preconditions for Unsafe Acts
        Supervisory Factors
          Organizational influences



     Unsafe Acts
     The unsafe acts of operators can be classified as either an error
     or violation. In general, an error represents normal accepted
     behavior that fails to achieve the desired outcome. Violations,
     on the other the result of intentional departures from practices.
     The operative term here is “International” meaning the
     individual knew the accepted practice yet chose to disregard it.



     Preconditions for Unsafe Acts
     To fully understand the causal chain of events, one must dig
     deeper into why the unsafe acts took place, essentially asking:
     "How was this day different from any other?"
     The process involves analyzing preconditions of unsafe acts,
     which include situational factors, the condition of the operator
     and personnel factors
     .
