Supervisory Factors
In most instances, supervisors (middle management) had an
opportunity to influence employee behavior and conditions.
Therefore, no investigation would be at least considering what,
if of front-line supervisors impacted the of the event.
Organizational Influences
In some cases, decisions made at the very top of the
organization by senior leadership can influence behavior
at the pointy end of the spear.
Unfortunately, causal factors at this level are the most difficult
to uncover yet arguably the genesis of many adverse events.
