VIOLATIONS
Routine Violations
Routine Violations involve the intentional "bending" of the rules
and are often habitual deviations from the rules that are tolerated
by management.
● Shortcuts -working - around established protocols,
intentionally skipping steps in a procedure
● Inappropriate use of tools/technology - disabling alarms,
removing safety guards
● Disregarding orders/directions - ignoring supervisor's
instructions, noncompliance with safety warnings

Exceptional Violations
Exceptional Violations are isolated deviations from the rules but
are not indicative of one's behavior.
These violations are not tolerated by management.
● Unqualified actions - performing activities without
Incense/credentials
● Disruptive behavior - arguing, physical altercations,
abusing equipment
● Excessive risk-taking actions - that pose unreasonable
risk of harm
