Every veteran with an honorable discharge qualifies.

Process begins with a consult from primary care to occupational therapy for a home safety evaluation.

Once evaluation is completed, a HISA consult is sent to prosthetic.

A Pre-approval letter will then be sent out with a HISA application packet

Amount of benefit is determined by the service connection disability percentage or the medical justification per the consult.

Service connection 50% or higher qualifies for $6,800.00

Service connection of less then 50% or non-service connected qualify for $2,000.00

Examples of approved modifications:

• Permanent ramps for ingress/egress of residence • Door latches

• Access to kitchen or bathroom sinks or counters • Electrical outlets and wall switches
• Entrance pathways or driveways • Handrails (metal or wood)

• Updates to electrical systems and whole-house • Lowering of windows

generators to support of Life Sustaining Equipment • Modifying of kitchen or bathroom counters
• Central air conditioning • Window tinting or shading

• Hardwood flooring • Roll-in, barrier-free showers, or conversion from

bathtub to seated shower
