PART 2 - Question and Answer Section
Risk Assessment – 58Q – Working at Height and the use of Ladders..
This record is to be kept on the driver's file.
To assess the understanding of this risk assessment and the safe system of work (RASSoW) the driver carrying out this assessment must answer the questions correctly to
show that they have understood what is required to carry out this task safety. Any questions answered incorrectly will be explained in more detail by the person rolling
out this training. This is not a test. It's an assessment. Read the question and place A, B, C or D in the white box.

Question 1 – Gaining entry and exiting a vehicle cab. Drivers should not use: Question 5 – Fall prevention. One of the ways we can prevent a fall is by:
A. Wearing your safety boot.
A. 3 points of contact whilst entering a vehicle.
B. The steering wheel as a handle when entering the vehicle cab. B. Listening to your colleague.
C. Doing strength and balance exercises.

Question 2 – Gaining entry and exiting a vehicle trailer: Under NO circumstances are Question 6 – Assure App. Always report a:
drivers to:
A. To report any near misses via the Assure app. A. Near Miss.
B. Black vehicles with low pressure tyres.
B. Fill up the vehicle with diesel at the end of the working day.
C. Low battery use on your company phone.
C. Jump off the back of a trailer.
D. Not wearing none slip boots with a steal toe cap.
D. Wear PPE whilst working.
Question 3 – Gaining entry and exiting a vehicle trailer: Drivers are reminded that Question 7 – Fall Prevention. If you do fall or gain an injury, then you must complete
when entering the trailer to clean behind the headboard they must comply with: the Assure app and:
A. The rules of the official highway code. A. Call Lincs Commercials.
B. Inform the transport office.
B. The instructions printed on the back of the door.
C. Wearing a balaclava. C. Park the vehicle up. RA-58Q
D. Complete a defect report.
D. Wearing five pieces of PPE are to be worn.
Question 4 – Entry to a compactor. Drivers must not enter: Question 8 – Collisions with vehicles and plant. Do not just get out of the cab or Access and Egress to
trailer WITHOUT firstly: Vehicles & Trailers
A. Compactors, skips or RO-RO bins. A. Selecting the correct mode on the tachograph machine.
B. Delivering or collection your next load.
B. Passwords to access the LMS (Learning Management System)
C. The vehicle wash on Mondays. C. Dropping off a skip or bin.
D. Taking that extra 3 seconds to look about.
D. Manual entries when required in a tachograph machine.
