e. Tax Identification number (TIN)
f. Statement of Account from Social Security System (SSS)
g. Pag-IBIG Number in the Member Data Form (MDF)
h. Philhealth number in the Member Data Record (MDR)
i. Xeroxed Copy of Latest Community Tax Certificate (cedula) Update
All information is subject for verification. The school reserves the right to not
consider applicants found to have falsified his/her Personal Data Form. Only after the
completion of all the employment requirements will a new hiree officially start receiving
his/her salary.
iv. Placement. Once the recommendation is approved, the HRDM officer issues an
employment contract or appointment letter duly approved by the by the School
Director and the President and signed/conformed with by the new personnel. The
employment contract or appointment letter is then distributed as follows:
a) Original copy – employee’s file (201)
b) Duplicate – Employee’s copy
