ADMISSION REQUIREMENTS
1.INCOMING FRESHMEN
An incoming freshmen shall be required to submit the original and photocopy of the
following:
✓ High School Card
✓ Certificate of Good Moral Character
✓ Authenticated Copy of Birth Certificate (PSA)
✓ Medical Certificate
✓ Certificate of Residence from Brgy. Chairman
✓ 2 pcs. 2x2 ID Picture (white background)
✓ Result of Admission Test
✓ Standardized Aptitude Test/College Qualifying Test Result
✓ Result of Interview
✓ 2pcs long brown envelopes
2. TRANSFEREE
A transferee is required to submit the original/photocopy of the following:
✓ Certificate of transfer Credential (Honorable Dismissal)
✓ Transcript of Records (TOR) for reference
✓ Certificate of Good Moral Character
✓ Authenticated Copy of Birth Certificate (PSA)
✓ Medical Certificate
✓ Certificate of Residence from Brgy. Chairman
✓ Parent's Income Tax Return (ITR)
✓ 2 pcs. 2x2 ID Picture (with white Background)
✓ Result of Admission test
✓ Result of Interview
✓ GPA of 85% and above for those seeking admission in courses which require
licensure examination (with no failing grade)
✓ 2 pcs long brown envelopes
