• Complete biometric screening and meet 3 of 5 benchmarks.
• Complete health assessment.
• Earn $30 per pay reduction on following plan years’ medical plan premium.
Activity Health Points
• Earn health points throughout the year by completing designated wellness
activities during the year.
• May earn up to a $175 gift card.
