PetVet offers eligible employees and their family additional benefits to enhance

Voluntary Benefit Options
Critical Illness Insurance An illness can seriously affect your financial health. Critical Illness Coverage through
Aflac can offset the costs your existing health and disability benefits do not cover. You
may elect coverage for you, your spouse and/or children, which will be paid in a lump
sum in the event you are diagnosed with certain conditions, including (but not limited to)
a heart attack, stroke and major organ failure.
Accident Insurance Group Voluntary Accident Coverage through Aflac provides cash benefits for out-of-
pocket expenses associated with an accidental injury and can help protect your savings
should an accident occur, whether on or off the job. This policy pays you a set amount
per event, such as hospital admission, surgery, and even outpatient doctor visits.
Coverage is available to you and your family members.
Hospital Indemnity Insurance through Aflac pays a daily benefit if you have a covered
Hospital Indemnity Insurance stay in a hospital, critical care unit or rehabilitation facility. The benefit amount is
determined based on the type of facility and the number of days you stay. Coverage is
available to you and your family members.
Legal Group Legal through MetLife Legal provides unlimited access to professional legal
assistance through the ease of one toll-free number and a nationwide network of
professional, qualified attorneys. Attorneys are available via phone consultations, office
visits or email. Additional services outside the scope of the plan are offered at reduced
rates by participating attorneys.
Student Loan Refinancing PetVet is pleased to offer qualifying employees and their friends and family a student
loan consolidation and refinancing service. Administered by SoFi, the program reduces
your loan repayments by consolidating and refinancing your federal and private loans
