Life and AD&D Insurance
Impact Floors provides basic life and AD&D insurance for Voluntary Life and AD&D Insurance
employees and offers voluntary insurance options for You may choose to purchase additional life and AD&D
employees and their dependents. This insurance is offered coverage for yourself and your dependents at affordable
through MetLife group rates. Rates are based on age and the coverage
level chosen.
Basic Life and AD&D Insurance
Life insurance is an important part of your financial wellbeing, MetLife Voluntary Life and AD&D Insurance for you
especially if others depend on you for support. Impact Floors Employee
provides basic life and accidental death and dismemberment
insurance at no cost equal to $15,000. Coverage is automatic; Increments of $10,000 up to 5x your base annual salary
you do not need to enroll. Up to a $500,000 maximum
Guaranteed issue up to $100,000
Family Medical Leave Act (FMLA) Voluntary Life and AD&D Insurance
for your dependents
If you have been with the company for 12 months, Spouse Child(ren)
you may be eligible for up to 12 work weeks of unpaid
leave per year under the Family and Medical Leave Act Increments of $5,000 Children under 15 days old:
