About Your Benefits

In an effort to offer the best in benefits to our employees, we are pleased to continue providing supplemental benefits through
Aflac. As the world’s largest provider of voluntary, pre-tax supplemental benefit plans, Aflac provides cash dollars to more than 40
million people when they or a family member face a serious illness or accident. Aflac allows you to customize your benefit package.
Each employee will have the opportunity to assemble a package of benefits meaningful to his or her personal situation. Below is a
brief summary of the programs offered.

EZ Shield
Fraud is a real concern. But now there’s a real solution. Aflac Fraud Protection helps ease your worries — giving you
more time to focus on what matters most to you. Service includes online data vault, internet monitoring and restoration
services.

Accident Plan
The Accident plan ensures, unlike pure disability, that you do not have to be disabled to have an accident claim paid.
Benefits include: emergency room, follow-up treatments, hospital income benefit, lump sum injury benefits, appliances,
physical therapy, ambulance service, lodging, and even wellness incentives. Family coverage is available on this plan.

Cancer Plan
Although health insurance can help offset the costs of cancer treatment, you will most likely need to pay deductibles
and copayments on your own. Additionally, cancer treatment can cause out-of- pocket expenses that aren’t covered by
traditional health insurance. Meanwhile, living expenses continue as well. Aflac helps provide an important safety net in
fighting the consequences of cancer that result beyond traditional health insurance. Family coverage is available on this
plan.

Hospital Plan
The Hospital plan is designed to pay a fixed dollar amount each day that a covered person is confined in the hospital as a
result of a covered sickness or injury. Benefits include initial hospitalization, daily hospital confinement and ambulance
benefits, as well as rehabilitation and even a short stay hospital benefit. Family coverage is available on this plan.

Lump Sum Critical Illness Plan
The Lump Sum Critical Illness Plan makes coverage available, in the form of a large, lump sum benefit, for specific health
incidents such as a heart attack, stroke, paralysis, etc. Family coverage is available on this plan: spouse and dependents
are covered at the same benefits amounts as the primary insured.

Personal Disability Income Protector
Protect your income with a plan tailored to your needs. Aflac’s Personal Disability Income Protector Plan may help
provide you with a source of income if you become disabled due to sickness or off-the-job injury.
