Eligibility You are considered eligible for benefits under this program if
you are a full-time employee of Skyworks working 20 hours or
more per week. New Hires are eligible for benefits effective
upon date of hire so long as they are full-time employees
working 20 hours or more per week. Interns and temporary
employees are not eligible for benefits.

Dependents
Eligible dependents include your spouse and children up
to age 21 or up to age 25 if a full time student. Dependent
verification documents are required to add a dependent to
your benefit plans.

Domestic Partnership
Skyworks recognizes that families come in all shapes and
sizes and offers domestic partner coverage. A domestic
partner must be financially interdependent, living in the same
quarters and in a committed relationship at least 6 months.
To qualify, an Affidavit of Domestic Partnership is required.
In the event you wish to remove your domestic partner from
your benefits plans, please complete the Domestic Partner
Termination form.
