                                   With that in mind, the ECRM Benefits Plan is designed to recognize the diverse needs of
                                   our workforce. Our plan continues to:
                                        • provide competitive and comprehensive benefits options that allow you to make
                                          choices;
                                        • maintain a program that considers individual needs;
                                        • offer plans to provide long-term financial security for you and your family.

                                   We encourage you to review all your options before making your benefit elections. Only
                                   you can determine which benefits are the best fit for you and your family. We want you to
                                   understand all your options and make informed decisions.

                                   Benefit Basics
                                   Once you elect your ECRM benefit options, your elections remain in effect for the benefit
                                   plan year. You may only change coverage during the plan year due to a qualified "life
                                   event" and must do so within 31 days of the event. If there isn’t a qualified "life event"
                                   then benefit changes can be made during annual open enrollment. ECRM encourages
                                   you to review all your benefits and dependent coverage eligibility at open enrollment and
                                   contact Human Resources if you have questions or if you would like to make changes.


                                   Life Events


                                        • Change in status, which include:  marital, number of dependents, employment,
                                          dependent satisfies or ceases to satisfy eligibility requirements
                                        • Significant cost or coverage changes to dependent’s benefits
                                        • Changes due to judgment, decree or court order
                                        • Entitlement to Medicare or Medicaid
