VOLUNTARY LIFE AND AD&D INSURANCE
Your Voluntary Life and AD&D Insurance - Unum
You have the option to purchase additional life insurance coverage. New hires can elect Voluntary Life
Insurance for themselves and their spouses up to the guaranteed issue amounts listed with no medical
underwriting. Employees who did not elect coverage when they were originally hired can still take
advantage of this coverage but will have to provide Evidence of Insurability information and be
medically underwritten.
Benefit
Evidence of Insurability
Employee Increments of $10,000 up to 5 times your annual salary, not to exceed $500,000
If you apply for additional life insurance, you may be required to fill out evidence of insurability, also called an EOI form.
Life (EOI required for amounts over $200,000)
After the enrollment, a member of the Human Resource team will send you a digital evidence of insurability form to complete.
The form should be returned directly to Unum. The amount you elect that is more than the Guaranteed Issue limits will

Voluntary Employee and Spouse Life insurance rates are based on employee age bands. The Employee’s age is updated the
month after their birth month.
To carry Voluntary Life Insurance and/or Voluntary AD&D for your Dependents you must elect Voluntary Life Insurance and/or
Voluntary AD&D for yourself.
Evidence of Insurability:
If you apply for additional life insurance, you may be required to fill out a medical questionnaire called an EOI form. The
form should be returned directly to Unum. The amount you elect that is more than the Guaranteed Issue limits will not be
effective until you are approved for coverage.
Adding Dependent Coverage:
If you do not add your dependents when you are initially enrolling, they cannot be added unless you experience a
Qualifying Life Event. This event must correspond with the dependent you are adding such as birth of a baby will allow
you to add the dependent child only or getting married will allow you to add your new spouse only.
