Eligibility: How Does it Work?
Who’s Eligible?
You. If you are a regular employee who works 30 or more hours per week, you can participate in the benefits described in this guide.
Your coverage begins on your hire date. You can enroll as a new hire or you can enroll during the annual Open Enrollment period each
year. If you are a new hire or are newly eligible for benefits, you must enroll within 30 days of your initial eligibility date. In order to
comply with the Affordable Care Act (ACA), Clari determines your eligibility for medical coverage based on the number of hours you
work each month.
Your eligible dependents. If you want to enroll your dependents for coverage, you will need to register them by providing their names,
date of birth and social security number. You must enroll your dependents in the same plans in which you are enrolling for the year.
Your Eligible Dependents Are:
• Your spouse (as defined by applicable state law) • Your stepchildren whom you support and who live with you
• Your same-sex or opposite sex domestic partner, please note in a parent-child relationship
imputed income will apply • Children placed in your home for adoption
• Your unmarried, dependent children up to age 26 • Any other children you support for whom you are the legal
• You or your domestic partner’s natural or adopted children guardian or for whom you are required to provide coverage
who can be claimed as your tax dependents as the result of a qualified medical child support order

You cannot change your benefits during the year unless you experience a qualifying life event.
