4. You will then be asked to add your Beneficiaries (NOTE: you are required to enter beneficiaries
otherwise you will not be able to submit your enrollment).

5. If you are enrolling your Spouse, Domestic Partner or Children, you must enter then in the Current
Dependent Information and identify them accordingly. If you do not provide this information, you
will only be provided with Employee ONLY coverage options and will not be able to enroll your family
members into any of the plans.
