c. If you are enrolling to the United Health Care – Canopy HMO Plan, you must select a
Primary Care Provider (PCP) and provide the 10-digit PCP code when you enroll in the
UnitedHealthcare Canopy HMO Plan. Provider ID number is a 10-digit number available
on myuhc.com when searching for a network PCP. Please make sure to use the PCP
number and not the NPI number that may also be listed. A PCP will be automatically
assigned to you if you don’t select one at enrollment. To enter the 10-digit PCP code,
click on SHOW DETAILS once you have selected the UCH- Canopy HMO plan, enter the
necessary information under Relationship & Managed Care Provider. Don’t forget to click
