Summary of Material Modifications

This summary of material modifications (this “SMM”) supplements the Major League Baseball
League-Wide Insurance Program Plan (the “Plan”) and Summary Plan Description (the “SPD”),
which was last updated on January 1, 2015. This SMM is intended to be read as part of the SPD
and is provided for your information and review. No other action is necessary on your part.
Please keep this SMM with the SPD for further reference.

The following changes were effective beginning January1, 2016.

Changes to the list of Baseball-Related Entities

The list of “Baseball-Related Entities” in the Introduction section is amended by adding the
following entities:

Boston Red Sox (The Boston Red Sox Baseball Club, L.P.)
Lerner Corp.
New York Mets (Sterling Mets, L.P.)

Changes to the Agent For Legal Process

The “Agent for Legal Process” in the section entitled Additional Information is replaced in its
entirety as follows:

“The agent for the service of legal process for the Plan is Steven Gonzalez, 1271 Avenue of
the Americas,New York, NY 10020.”

Changes to Appendix A

If your employer made changes to the eligibility provisions or changes were made to the benefit
options available to you, a new Appendix A will be attached. The new “Appendix A” replaces
the prior version in its entirety, if applicable.
