BENEFIT BASICS
Fort HealthCare offers a comprehensive suite of benefits to promote health and financial security for you and your family.
This booklet provides you with a summary of your benefit. Please review it carefully, it contains information to help you
choose the coverage that’s right for you and your family.
Eligibility
Most employees are eligible for the benefits described in this guide if their FTE is .5 (20 hours) or greater. Once per year,
during open enrollment period (usually in April), employees have the opportunity to enroll or make changes to their

benefits for the following plan year. Any changes made during the open enrollment period begin on May 1 and remain
in effect until April 30 the following calendar year.

▪ Health, Dental, Vision & Flexible spending Benefits are effective on the first day of the month following 30-days
employment or becoming benefit eligible.
▪ All other benefits are effective the first of the month following 90 days employment or becoming benefit eligible.
You may enroll your eligible dependents for coverage once you are eligible. Your eligible dependents include:

Your children up to age 26
Once your benefit elections become effective, they remain in effect until the end of the plan year (Apr. 30). You may
only change coverage within 31-days of a qualified life event.

You must notify Human Resources within 31-days of the qualified life event. Depending on the type of event, you may
be asked to provide proof of the event. If you do not contact Human Resources within 31-days of the qualified event,
you will have to wait until the next annual enrollment period to make changes (unless you experience another qualified
life event).
