Are Your Beneficiaries Up to Date?
Be sure to confirm or update your beneficiaries in Workday at least once a year. When you check or
update your life insurance beneficiary, don’t forget to also log in to Payflex at Payflex.com and Fidelity
at 401k.com to align your 401(k) and HSA beneficiaries coverage.

Disability Insurance

Disability insurance provides income replacement should you become disabled and unable to work
due to a non-work-related illness or injury. As shown below, OpenX provides eligible associates with
disability coverage at no cost.*

Coverage is automatic: You do not need to enroll.

COVERAGE BENEFIT

Short-Term Disability 60% of your weekly base salary, up to a maximum of $2,308
per week for the first 25 weeks of a disability after a one-week
waiting period.

Long-Term Disability 60% of your salary, up to a maximum of $15,000 per month if
you are disabled and are unable to work for more than 180
days.

If you die while receiving benefits, a survivor benefit will be paid
to your lawful spouse, and eligible children of estate. The plan
will pay a single lump sum equal to three months of benefits.
If you have a pre-existing condition that is present three
months prior to your effective date, LTD may not be covered in
your first 12 months under the plan.
