Vision plan – UnitedHealthcare
The vision plan provides coverage for routine eye exams and pays for all or a portion of the cost of glasses or contact
lenses. You can choose any provider; however, you always save money if you see in-network providers. We offer a vision
plan option through UnitedHealthcare.

Routine/Comprehensive Eye Exam $10 Copay Up to $40
Lenses – Single/Bifocal/Trifocal, once every 12 months $10 Copay Up to $40/$60/$80
$130 Allowance + 30% over
Frames – 1 pair of frames every 12 months Up to $45

Contacts (Elective) – every 12 months $125 Allowance Up to $100
Medically Necessary Contacts $0 Copay Up to $210
Plan includes a second exam for children under age 13 and pregnant or breastfeeding women (after applicable copayment).
