helps our employees stay healthy and feel secure while maintaining a work/life balance.

Advanced Neuro Solutions offers a NEW choice of two (2) comprehensive Major Medical
plans with United Healthcare (UHC) of Oklahoma. Both Medical plans come with $0 Virtual
Doctor Visits and comprehensive annual preventative care benefits covered 100% along
with other Wellness programs for healthier lifestyles. Major Network differences are ex-
plained below:

• The United Healthcare Exclusive Provider Organization (EPO) plan operates similar to an
HMO, with in-network benefits only, using the nationwide UHC Choice Network, with No
PCP or Referral Requirements.

• The United Healthcare Preferred Provider Organization (PPO) plan is a standard plan

with In & Out of Network benefits, using the nationwide UHC Choice Plus Network.
We provide 100% Employer Paid Dental, Vision, Basic Life and AD&D for full-time employees.
Advanced Neuro Solutions also contributes a major portion towards your medical. Dental
NOW has Adult Orthodontics Included.

helpful information, cost and necessary contact information.
Our benefit guide will not serve as an ID card or verification of your benefits at a provider’s

office, but it can provide assistance if needed. Refer to page 18 for instructions on how to
login to your customized Employee Benefit Portal that has policy and carrier information
