Heritage School of Texas is committed to a comprehensive benefit program that helps you
stay healthy and feel secure while maintaining a work/life balance.

Heritage School of Texas is pleased to offer two (2) NEW Baylor Scott & White Medical plan
options to meet your individual needs. Both are Nationwide PPO Plans with in and out of
network coverage. We also offer our full-time employees Voluntary Dental and Vision
through Humana. Heritage School of Texas pays a sizable flat dollar amount of full-time em-
ployee’s cost for the medical, dental or vision plan of your choosing.

Heritage School of Texas is concerned about your financial security and for that, we offer
an option to enroll in Voluntary Short Term Disability (STD) and Voluntary Long-Term
Disability (LTD). We also provide all Full-Time employees with a flat amount of $25,000 Basic
Life insurance with Accidental Death and Dismemberment (AD&D).

Please retain a copy of this guide in order to review benefits, cost, and contact
information. This booklet will not serve as an ID card or verification of benefits, but can
provide assistance if needed. This year we are launching a NEW benefits administration
portal, Employee Navigator. You will receive a Welcome email with login instructions. You
may access the site 24/7 with your Personal ID and Password once your benefits have been
set-up following open enrollment (see instructions on page 19).
