Important Information





                                         Who is Eligible?


                                         If you are a full-time employee working a minimum of 30 hours
                                         per week, you are eligible to enroll in the benefits described in
                                         this guide. Eligibility starts the first of the month following date
                                         of  hire.  Family  members  are  eligible  for  all  lines  of  coverage
                                         (see benefit page for dependent children age requirements).





         When to Enroll

         All  employees  are  required  to  complete  their  New  Hire  and  open
         enrollment  elections  online  via  your  NEW  Employee  Benefit  Website,
         Employee Navigator. Instructions on how to do so will be sent to you
         electronically and can also be found on page 19.
         If you have any questions, please contact your School Administrator,
         Maegan Marshall, for assistance. Open Enrollment this year is in May
         for a June 1st effective date.

         Can I make changes during the plan year?
         Yes, you have HIPAA Special Enrollment Rights. To review these rights,

         please refer to page 23.




          LAW TO HAVE HEALTH INSURANCE


          Individuals are still required to have medical unless you have qualified waivers as mandated by
          current law. The penalty fine for not having coverage ended January 1, 2019.


















              Please note that the medical plans offered by Heritage School of Texas exceed the

             minimum  value standard for health coverage according to the Affordable Care Act.
