          Revelations  Health  Care  Group  is  pleased  to  offer  our  full-time  Group  1  employees  Three  United
          Healthcare Major Medical Options and two Pan American Limited medical lower cost options with
          Healthcare2U  (DCP)  Direct  Primary  Care  benefit  that  offers  Unlimited  $10  PCP  Doctor  Visits  and
          Unlimited  $25  Urgent  Care  visits,  see  pages  14  &  15  for  more  information.  Both  UHC  and  Pan
          American  have  Unlimited  Preventative  Care  coverage  with  NO  Copay,  Deductibles,  or  Out  of
          Pocket Costs. Our benefit package includes Voluntary Dental, and Vision with Mutual of Omaha.
          Low cost Voluntary Life with matching Accidental Death & Dismemberment. Please Note New Hires
          (during initial enrollment) can apply for up to $150,000 with NO Health Questions, Spouses $30,000
          and Children $10,000.  Also new this year  is Critical Illness with NO Health Questions meaning you
          cannot be turned down. Mutual of Omaha  On and Off the Job Accident Coverage for employees
          and  dependents.          NEW  this  year  we  have  added  Voluntary  Short-Term  Disability  (STD).
          Revelations Health Care Group contributes a portion of the monthly employee only premium for all
          of the Medical plan options.


          Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
          necessary contact information. This booklet will not serve as an ID card or verification of benefits at
          a provider’s office, but can provide assistance if needed. Refer to page 33 for instructions on how
