         Hanover Medical Management Services, LLC is committed to a comprehensive benefit program that helps our employees stay
         healthy and feel secure while maintaining a quality work/life balance.

         Hanover Medical Management Services, LLC is pleased to continue offering 100% Employer Paid Life Insurance with AD&D and
         Long-Term  Disability  (LTD)  to  all  employees.  We  also  offer  a  wide  variety  of  voluntary  benefits  including  dental,  vision,  and
         employee, spouse or child Voluntary Life with AD&D. Your medical plans are addressed by each individual company.

         During Open Enrollment, currently enrolled employees may increase their Employee Voluntary Life by $10,000 up to the Guaran-
         teed Issue (GI) of $150,000, Spouse Voluntary Life by $5,000 up to Guaranteed Issue of $25,000, or Child Voluntary Life up to the
         $10,000 Guaranteed Issue Max. We are offering dental and vision plans that are enhanced and low cost. Our Dental plan even
         includes a $1,500 Lifetime Max of Orthodontia for children under the age of 19.

         All medical benefits will be managed at a different time. You will receive separate benefit and enrollment  information when
                                        open enrollment for your medical insurance begins.

         For comprehensive information about your benefits, please visit your NEW Employee Benefits Website Employee Navigator,
         anytime 24/7. This personalized portal offers detailed information on your current plan, benefit summaries, cost breakdowns,
         provider and network search tools, instructional videos, carrier links, and relevant documents. Please refer to pages 15-16 of this
         guide for detailed login instructions and benefit selection procedures.


         Please retain a copy of this guide for future reference, including coverage benefits, cost information, and essential contact details
         provided on page 17. Note that this booklet does not function as an ID card or proof of benefits for providers but can assist you with
         relevant information as needed.
