Important Information








                                           Who is Eligible?
                                           If  you  are  a  Full-Time  employee,  working  a  minimum  of  30  hours  per
                                           week, you are eligible to enroll in the benefits described in this guide.

                                           New Employees are eligible on the first of the month following their date
                                           of hire.
                                           Family members are eligible for all lines  of coverage except employer
                                           paid life/AD&D and long-term disability (see benefit page for depend-
                                           ent children age requirements).



           When to Enroll
           All employees are required to complete the New Hire Enrollment paperwork at
           new hire and once a year during the Annual Open Enrollment.

           Open enrollment is in December for a January 1st effective date.
           New hire enrollment forms should be submitted to Wendy Perry
           with Human Resources within the first 30 days of employment.
