         National Door Industries, Inc. is committed to a comprehensive benefit program that helps our employees stay healthy
         and feel secure while maintaining a quality work/life balance.

         National Door Industries, Inc. is pleased to offer a choice of two (2) NEW  BCBSTX  major medical plans with improved
         benefits and $0  Virtual  Visit  Copays.  The  NEW BCBS  Plan  Annual  Out  of  Pocket  Maximum  (OOP)  includes  all
         Deductible, Copays, Coinsurance, etc. This year your  Employee Only Rates are 42% less for the Core Plan.
                            National Door contributes 75% of the cost for the employee only core plan.

         We are continuing to offer two (2) Humana PPO Voluntary Dental Plan options. Both dental plans have NO  ANNUAL
         Maximum  and enhanced benefits. We also offer Voluntary Vision through Superior  Vision  and a flat dollar amount of
         100% Employer Paid Life Insurance with matching Accidental Death and Dismemberment through Mutual of Omaha.

         NEW Allstate Voluntary low cost Accident,  Critical  Illness  and  Short  Term  Disability summarized on pages  16-20.
         Critical Illness includes Cancer along with other critical illnesses with no medical questions. You cannot be turned down
         for any of these plans.

              We pay 100% for ALL Employees to have access to 24/7 Telehealth phone or video Virtual Visits through
          HealthiestYou. This service may be used whether you are enrolled in our BCBS Medical plan or not. This service
              may be used by you and your dependents.  HealthiestYou is a free resource, but you must sign up for it.

         For comprehensive information about your benefits, please visit your NEW Employee Benefits Website Employee
         Navigator, anytime 24/7. This personalized portal offers detailed information on your current plan, benefit summaries,
         cost breakdowns, provider and network search tools, instructional videos, carrier links, and relevant documents. Please
         refer to pages 24-25 of this guide for detailed login instructions and benefit selection procedures.

         details provided on page 26. Note that this booklet does not function as an ID card or proof of benefits for providers but
         can assist you with relevant information as needed.
