Long Term Disability: Equitable
Employer Paid (LTD) Long-Term Disability Coverage
Gas Clip Technologies provides employees with Long Term Disability (LTD) coverage intend-
ed to protect your income in case you become ill or injured and unable to work.
• Benefits begin 90 days after an injury or illness
• Benefits can continue up to 5 years (graded).
• Benefit amount pays up to 50% of your pre-disability monthly income
• Maximum Monthly Benefit paid is $3,000 (taxed)
• Pays for Total and Partial Disabilities caused by accidents and sickness

Definition of Total Disability:
You are prevented from performing one or more of the Essential Duties of:
1) Your Occupation during the Elimination Period;
2) Your Occupation, for the 24 months following the Elimination Period, and as a result Your Current Monthly
Earnings are less than 80% of Your Indexed Pre-disability Earnings; and
3) after that, Any Occupation.
Partially Disabled:
If you become disabled and can work part-time (but not full-time), you may be eligible for partial disability
benefits.
Pre-Existing Condition Limitations:
Yes. 3 months prior /12 months after.
