         Texas Flood, LLP, is committed to a comprehensive benefit program that helps our employees stay healthy
         and feel secure while maintaining a quality work/life balance.

         MEDICAL We offer a nationwide major medical plan through Assured Benefit Administrators (ABA). Our ABA
         Medical  plan  is  a  PPO  plan  with  In  and  Out  of  Network  benefits  through  UHC  Choice  Network.  $6,000
         Calendar Year Deductible (CYD) and $40 Primary Care office visit and $75 Specialist office visit copays.

           Virtual Visits are facilitated by “Lyric Virtual Visit Service” and are a $0 copay; 24 hours / 7 days a week.

         DENTAL We are continuing to offer our comprehensive Dental coverage through UNUM. Our Dental plan is a
         PPO and has $1,000 annual maximum benefits. Our plan reimburses non-network dentists at the Maximum
         Allowable Charge (MAC), therefore; choosing an In Network Dentist will provide you the best / lowest cost.
         Preventative services are covered 100% with no deductible.

         VISION We are also continuing to offer our comprehensive Vision coverage through UNUM. Our Vision plan
         offers  $10  Exam  Copay,  $10  Materials  Copay  and  a  $130  allowance  for  Frames  or  Contacts  in  lieu  of
         glasses.

         (Employee Navigator), anytime 24/7. This personalized portal offers detailed information on your current
         plan, benefit summaries, cost breakdowns, provider and network search tools, instructional videos, carrier
         links, and relevant documents.

         instructions and benefit selection procedures.

         of benefits for providers but can assist you with relevant information as needed.
