Voluntary Short Term Disability (STD):

         Voluntary (STD) Short-Term Disability Coverage


         US Neuro, LLC, provides all full time employees the option to purchase Voluntary Short-Term
         Disability (STD) coverage intended to protect your income for a short duration in case you
         become ill or injured at low cost group rates.

          •  Benefits begin the 1st day following an accident, and 8th day following an illness / pregnancy.

          •  Benefits can continue up to a maximum duration of 13 weeks

          •  This policy will cover 60% of your weekly base income up to $1,500

          •  Cost are after tax and Weekly Benefit are Income Tax Free

          •  Pays for Total and Partial Disabilities caused by accidents and illness
         Definition of Disability:
           Total Disability means that, due to an injury or sickness, an employee is unable:
                1. During the ELIMINATION PERIOD and the "OWN OCCUPATION" Period, to perform each of the main
           duties of his or her regular occupation; and
                2. After the "OWN OCCUPATION" Period, to perform each of the main duties of any occupation which
           his or her training, education or experience will reasonably allow. The loss of a professional license, an
           occupational license or certification, or a driver's license for any reason does not, by itself, constitute Total
           Disability.
         Partially Disability Benefits:

         A disabled employee can satisfy the elimination period with days of total disability, partial disability or a com-
         bination of both. An employee may qualify for a partial disability benefit if he or she is Partially Disabled and is
         under the regular care of a physician.
         PRE-EXISTING CONDITION:

           If you have a medical condition that begins before your coverage takes effect, and you receive treatment
           for this condition within the 12 months leading up to your coverage start date, you may not be eligible for
           benefits for that condition until you have been covered by the plan for 12 months.
         OTHER BENEFITS:

          •  Rehabilitation Assistance Benefit: Employees who participate in an approved rehabilitation program are
            eligible to receive an additional percent of benefit. Additionally, approved program costs may be
            reimbursed.
          •  Waiver of Premium: You will not be required to pay premium during any time of approved total or partial
            disability.
          •  Portability, you can take it with you
          •  Pregnancy—Normal Delivery—6 Weeks
          •  Pregnancy C-Section Benefit - 8 Weeks
