         US Neuro, LLC, LLC dba US Neuro, LLC is committed to a comprehensive benefit program that helps our employees stay healthy
         and feel secure while maintaining a work/life balance.

         We are NOW offering a choice between four (4) comprehensive Major Medical plans with Meritain Health using the  Aetna POS II
         Network. Our four Medical plan options include a low-cost Health Savings Account (HSA) option and three PPO plans with $2,000,
         $3,000, and $5,000 Deductible options. All plan designs have been enhanced, with costs remaining the same or decreasing across
         all tiers. Additional highlights include $0 virtual care TELADOC (health and behavioral), expanded wellness resources, Basic Labs/X-
         Rays will be paid 100% with no Copay or Calendar Year Deductibles (CYD), Imaging (i.e. CT, MRI’s, PET scans) will be copays with
         no CYD. You have access to the exclusive US Imaging Network® (USIN) more details on Page 8.

         We are transitioning to Lincoln Financial Group as our new ancillary carrier for Employer Paid Life, LTD and Voluntary Life, STD,
         Dental, and Vision.
         We are offering a choice between two (2) Voluntary Dental Options. One LOW Dental Plan with a $2,000 Calendar Year Max and
         one HIGH Dental plan with a $4,000 Calendar Year Max.. Non-Network Providers will be reimbursed at 90th Percentile as it is now.

         We provide $25,0000 in Employer Paid Life with Accidental Death and Dismemberment (AD&D) for all Full Time Employees at NO
         COST to YOU.  Voluntary  Employee  Life  is available at low cost term rates. Voluntary Life is Guaranteed Issue (GI) no health
         questions up to $180,000 (Employees), $35,000 (Spouse / Domestic Partners) and $20,000 (Children).  Both Employee and Spouse
         includes Accidental Death and Dismemberment (AD&D).
         We are providing Employer Paid Long-Term Disability (LTD) which will pay a cash benefit of 60% of your monthly income up to a
         Max of $7,000 were you to become disable and unable to return to work. Voluntary Short-Term Disability (STD) is available at low
         cost rates protecting 60% of your income.
         We are also providing Voluntary worksite Accident, Critical Illness, Hospital Indemnity and Permanent Universal Life through a
         partnership with Trustmark.
