Important Information
The intent of our benefit guide is to briefly highlight your benefits and NOT to replace your insurance
contracts or booklets. If this benefit guide does not address your specific benefit questions, please
refer to the Contact Information page 27 of this booklet. Your Employee Benefit Portal through
Employee Navigator includes plan information, summaries, costs, carriers contact information and
more. Please see the instructions on pages 25-26 to access this information.
Who is Eligible?
If you are a full-time employee, working a minimum of 30 hours you are eligible to enroll in
the benefits described in this guide. New employees are eligible on the first of the month
following date of hire. Spouses and dependent children are also eligible (see benefit page
for age requirements).
When to Enroll?
All employees are required to complete the New Hire online Enrollment through our
enrollment firm BenROLL at new hire and once a year during the Annual Open Enrollment.
BenROLL enrollment counselors will personally assist you in going over your
benefits and enrolling you in Employee Navigator.

Page 31 Open enrollment is in July for a August 1st effective date. New hire enrollment
login and password will be provided by Wendy Perry.
We also have online enrollment options. Refer to pages 28-29 for instructions on how to
login to your customized Employee Navigator Portal where you may make your elections
online and see all policy and carrier information and costs.
Can I make changes during the plan year? Yes
You have HIPAA Special Enrollment Rights. To review these rights, please refer to page 42.
LAW TO HAVE HEALTH INSURANCE
Individuals are still required to have medical unless you have qualified waivers as
mandated by current law. The penalty for not having coverage ended January 1, 2019.
Please note that all medical plans offered by US Neuro, LLC, exceed the minimum value
standard for health coverage according to the Affordable Care Act.
