Long-Term Disability:

Long-Term Disability Coverage
Summit LTC Management, LLC provides employees with Long-Term Disability coverage intended to
protect your income in case you become ill or injured.
• Benefits begin after 90 days after an injury or illness
• Benefits can continue up to Age 65 or Normal Social Security Retirement Age
• Benefit Amount pays up to 60% of your income. You may receive an additional 10% with Progressive
Income Benefit (PIB) if you have a loss of 2 or more (ADLs) for a Total Available Benefit of 70% based on
your income loss and/or definition of disability
• Maximum Monthly Benefit paid is $5,000 or more based on your income
• Additional 10% PIB benefit (up to $5,000) if you have a of loss 2 or more Activities of Daily Living
• Pays for Total and Partial Disabilities caused by accidents and sickness
DEFINITION OF DISABILITY
• Total Disability:
“During the Elimination and Own Occupation Period, it means that due to an injury or sickness the insured
employee is unable to perform each of the main duties of his or her own occupation”. The Own Occupation
period lasts for 5 years or more, after that you must meet the criteria for Disability from ANY occupation to
receive benefits until Social Security Retirement age.
• Partially Disabled:
“During the Elimination and Own Occupation Period, it means that due to an injury or sickness, the insured
employee: A: is unable to perform one or more of the main duties of his or her own occupation; or is unable to
perform such duties full-time; and B: is engaged in partial disability employment.”
• Progressive Income Benefit (PIB):
“The Progressive Income Benefit, which is standard on our LTD plans, automatically increases the core benefit
by 10 percentage points when an insured employee suffers a serious cognitive impairment or the loss of two
or more Activities of Daily Living (ADLs): bathing, dressing, using the restroom, transferring (to and from a bed,
chair or wheelchair), continence or eating.”
• PRE-EXISTING CONDITION:
If you have a medical condition that begins before your coverage takes effect, and you receive treatment
for this condition within the 3 months leading up to your coverage start date, you may not be eligible for
benefits for that condition until you have been covered by the plan for 12 months.
• Reasonable Accommodation - 50% up to $5,000
• Vocational Rehabilitation
• Family Income Benefit- $250 for up to 12 months
• Survivor Benefit - 3 Months
