         Summit LTC Management, LLC is committed to a comprehensive benefit program that helps our
         employees stay healthy and feel secure while maintaining a quality work/life balance.

         Summit LTC Management, LLC is pleased to offer our Group 1 employees medical options to meet
         your individual needs for yourself and your family. Our major medical which includes 100% covered
         with no out of pocket cost for Preventative Care (see page 4). 24/7 Virtual Doctor Visits at $0 Cost.
         To  help  offset  our  major  medical  deductible  we  have  an  Health  Reimbursement  Arrangement
         (HRA) that covers up to $3,000 annually.

         Summit contributes 100% of the cost for basic life and matching AD&D, (LTD) long-term disability.

         Our  benefit  package also includes voluntary  dental, vision,  life  with  AD&D,  voluntary  (STD)  short-
         term disability, Critical Illness and Accident.


         We provide employees and their family members with an Employee Assistance Program (EAP) and
         even a Worldwide  Travel  Assistance program.

         Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
         necessary contact information. This booklet will not serve as an ID card or verification of benefits at
