         (HRA):







                              Health Reimbursement Arrangement (HRA)


           In addition to your health insurance plan, Summit LTC Management, LLC also provides a
           Health Reimbursement Arrangement  (HRA).  An HRA is an employer-sponsored benefit

           plan designed to help pay a portion of your medical insurance deductible. You pay the
           first  $3,000  of  your  $6,000  In-Network  deductible.  Summit  will  contribute  the  second
           $3,000 to your HRA account which is reimbursable for eligible expenses incurred by you

           and/or your family to a maximum annual.

           You must be enrolled in your employer’s group health plan to qualify for the HRA.

           You CANNOT cash out the balance in your HRA.
