Summit LTC Management, LLC is committed to a comprehensive benefit program that helps our
employees stay healthy and feel secure while maintaining a quality work/life balance.
Summit LTC Management, LLC is pleased to offer our full-time Group 2 employees two MEC/ Limited
Medical Plan options to meet your individual needs for yourself and your family. NEW this year we
have added Healthcare2U Direct Primary Care benefit with Unlimited $10 PCP Doctor Visits and
Unlimited $25 Urgent Care visits, see pages 5-6 for more information. Your Medical Includes 100%
coverage for all your Adult and Children Preventative Care, see page 8. COVID-19 is covered 100%
during the COVID period. This is subject to change. Our benefit package also includes Voluntary
Dental, Vision, Life, AD&D, Voluntary Short-term Disability, Critical Illness and Accident coverage.
We also provide employees and their family members with an (EAP) Employee Assistance Program
and a Worldwide Travel Assistance program.
Summit LTC Management, LLC contributes a portion of the monthly employee only premium for
Plan 1 and 2. Summit LTC contribute 100% of the cost for your Basic Life and AD&D, EAP and Travel
Assistance Program.
Please retain a copy of this guide in order to review coverage benefits, cost information, and retain
necessary contact information. This booklet will not serve as an ID card or verification of benefits at
a provider’s office, but can provide assistance if needed. For additional contact numbers and
