ENROLLMENT PROCEDURE FOR

FRESHMEN AND TRANSFEREES

1 .Pay Entrance Examination Fee at the Accounting Office.
2.Take Entrance Examination at the Guidance Office.

3.Secure application form for admission (RegForm-01)
at the Registrar's Office and submit the following

documents:
Freshmen:

•Original copy of Form 138-A (High School Report Card)
•Certificate of Good Moral Character

•Certificate of Physical Fitness from the School Clinic
•NSO Live Birth Certificate

•1 pc. Long Folder
•1 pc. 2x2 ID Picture

•Marriage Certificate (for female married student)
Transferees:

•Certified true copy of TOR / Certification of Subjects
•Certificate of Good Moral Character

•Certificate of Physical Fitness from the School Clinic
•Transfer credentials from the school last attended

•NSO Live Birth Certificate
• 1 pc. Long Folder\

•1 pc. 2x2 ID Picture•
•Marriage Certificate (for female married student)

4.Accomplish RegForm-01 at their respective College.
5.Present RegForm-01 at the Registrar's Office for admission.
6.Proceed to Encoding Section.

7.Pay required down payment and claim Assessment Form

j at the Accounting Office.
8.Submit copy of RegForm-01 to the Registrar's and Dean's
Office

9.Proceed to Internet Room for ID picture taking,

10.CIaim Library Card at the Library.
