Explaining The Process To Employees
Educating Your Employees
The LOA process should be explained to employees as simply as possible. In this section we’ve provided a chronological, high-level overview and talking points for you to share with your employee.
LOA Process - Simplified
• The employee will need to file the leave request with The Standard Insurance company by either calling 800-232-3146 or filing online at standard.com/absence
• The Standard Insurance Company will review the LOA request and mail further instructions and required certifications to the employee’s home address
• All LOA requests must be verified and approved by the Standard (Except for Union specific leaves)
• Union specific leaves will need to be filed and processed by the local HR Manager
• The Standard Insurance Company will approve or deny a LOA request after receiving certification from either you and/or your treating physician
• The Standard Insurance Company will also review your eligibility for any salary continuation through Ventura Foods bridge pay program if your leave is related to your own serious medical condition
• The Standard Insurance Company will be in communication with you throughout your leave and it is very important that any document requests are responded to in a timely manner; otherwise your leave may be denied
• Before returning to work on an approved leave of absence for your own health condition: a Fitness-for-Duty/Return to Work Release form will be required to return back to work. Read, sign, have your doctor sign, and send in the Fitness-for- Duty/Return to Work Release form to Local HR and The Standard Insurance Company within 2 business days before you’re scheduled to return to work. This form is included in your leave approval packet from The Standard Insurance Company. If you do not have this form and need to get a copy, please contact The Standard Insurance Company at 800-232-3146 from 5 a.m. -
5 p.m. (PST), Monday through Friday.
• Before returning to work on an approved leave of absence for a NON health condition: contact Local HR within 2 business days before you’re scheduled return to work.
• After receiving return-to-work approval from the Ventura Foods, you must coordinate your work schedule with Local HR
