 Your Return to Work Checklist
Before you return to work, you MUST follow the steps outlined below.
 Notify Local HR at least 2 business days prior to the date you intend to report for work.
If you are on an approved leave for your own medical condition, a Fitness-for-Duty/Return to Work Release form will be required to return back to work.
Read, sign, have your doctor sign, and send in the Fitness-for-Duty/Return to Work Release form to Local HR and The Standard Insurance Company within 2 business days before you’re scheduled to return to work.
— This form is included in your leave approval packet from The Standard Insurance Company. If you do not have this form and need to get a copy, please contact The Standard Insurance Company at 1-800-232-3146 from 5 a.m. - 5 p.m. (PST), Monday through Friday.
You may not return to work unless your return has been scheduled with Local HR.
If the circumstances of your leave change and you are able to return to work earlier than the date indicated in recent communication with the Standard, please do the following:
Return completed Fitness-for-Duty/Return to Work Release form (if on an approved leave for your own medical condition) to local HR and the Standard.
Notify your local HR at least 2 business days prior to the date you intend to report for work.
Contact The Standard Insurance Company at least 2 business days prior to the date you intend to report for work.
If you have a disability or impairment that substantially limits one or more major life activities and want to consider a work accommodation for your return to work or required additional time, please contact The Standard Insurance Company as soon as possible.

You will not be able to return to work until The Standard Insurance Company has received and Ventura Foods has approved this form and your return to work has been coordinated by Ventura Foods local HR.
