verbal warning, in most instances, the associate should receive another verbal warning. Disciplinary Action Record forms, however, should never be removed from the associate’s personnel file.
Category I Rules
Violations of Category I rules are considered less serious in nature than those found in Categories II and III. Penalties for violating Category I rules may include:
First Offense Second Offense Third Offense
Fourth Offense
Documented counseling Written warning
Final written warning with or without suspension of up to five (5) working days without pay
Termination of employment
The penalty for an offense will vary with the circumstances, and while these guidelines should be applied uniformly, different levels of discipline may be applied, at the discretion of management, depending upon the circumstances. When violations are recurring or considered serious, any of these steps may be skipped.
1. Associates must maintain acceptable standards of respect for residents, families, vendors, visitors, team-leaders and co-workers and may not engage in gossip (other than conversations amongst themselves that are protected under any applicable union organizing laws).
2. Associates must follow location and departmental procedures.
3. Associates must perform the responsibilities of their position in an appropriate manner and at
assigned times.
4. Associates must promptly carry out general or specific instructions.
5. Associates must follow proper procedures regarding the time clock.
6. Associates may not start work before or continue to work after specified hours, or incur any
overtime hours, without prior approval of the team leader.
7. Associates must readily cooperate with fellow associates and/or team leaders.
8. Associates must comply with the dress code/personal appearance policy.
9. Associates must be at their workstation at the start of their shift and may not stop work before the
time specified.
10. Associates may not receive unauthorized visitors while on work time.
11. Associates must attend mandatory in-service, educational, and staff meetings.
12. Associates must follow safety, health, security and fire policies and procedures.
13. Associates may not leave the household or designated service area during their shift, without
communicating with their team and team leader.
14. Associates may not violate the location’s smoking/tobacco policy.
15. Associates may not loiter during work hours.
16. Associates must comply with a health review in accordance with State requirements.
17. Associates may not violate the bulletin board policy.
18. Associates may not create or contribute to unsanitary or unsafe conditions.
19. Associates may not neglect, waste or mishandle materials, machinery or other supplies.
20. Associates may not eat or consume food or refreshments in undesignated areas.
21. Associates may not enter The Piper at unauthorized times without proper permission.
22. Associates must call their team leader or the designated person at least two (2) hours prior to the
beginning of a shift for any day that they will not be present at work as scheduled.
23. Associates may not violate any other conduct or location work rule of a similar nature not
mentioned herein.
