paid back to the community in the event the associate terminates employment or is no longer eligible for PTO. All associates will be required to sign a payroll deduction authorization form authorizing repayment (see PTO Acknowledgement).
9. The Use of PTO in Conjunction with Unscheduled Absence. If the associate has PTO and has an unscheduled absence, the associate must use PTO for the absence. PTO must be used by non- exempt/hourly associates to receive compensation for all unscheduled absences. An associate who needs to use unscheduled PTO must notify the Leader/Mentor as soon as possible. The Leader/Mentor will then note the use of unscheduled PTO and submit to payroll for processing. The use of unscheduled PTO does not excuse the absence, and the associate may be subject to discipline and discharge in accordance with applicable attendance and/or disciplinary policies if the use of unscheduled PTO is deemed excessive. Associates who are on an approved LOA must use their PTO time in accordance with the relevant LOA policy.
10. Approval of PTO Requests. PTO is to be requested by submitting a written request to the associate’s Leader/Mentor and PTO must be used in order for time off to be approved. Approval of PTO requests is within the sole discretion of the Leader/Mentor and the Executive Director. Household and community operations will be a key consideration when reviewing PTO requests. The Community may set prime time (special projects, summer and holiday periods) PTO usage restrictions in order to assure appropriate staffing and to allow for associates to enjoy time with family and friends. When the PTO request is approved, it will be transmitted to the Business Office Manager for processing into Payroll.
11. Leave of Absence. PTO days must be used in conjunction with a personal or medical leave of absence, in accordance with the relevant LOA policy. (Please refer to The Piper FMLA policy and other LOA policies for minimum usage language and other guidelines.) In such circumstances, PTO is not considered a substitute for leave and cannot be used to extend a leave beyond the applicable time limits. An associate who takes a leave of absence will have his/her PTO accrual suspended for the time that they are on an unpaid leave, unless otherwise required by law.
12. PTO Policy Associate Acknowledgment. After reviewing a summary of the PTO Policy during the orientation process, each associate must execute the PTO Policy Associate Acknowledgment. The executed form will be retained in the associate’s personnel file.
13. Premium Pay Days. The attached schedule identifies Premium Pay Days, in which non-exempt associates will be paid time and one-half their base rate of pay for all hours worked. Associates are not eligible for premium pay if they take unscheduled time off on the scheduled workday before and/or after a specified holiday. These days are identified based upon the difficulties encountered in scheduling associates to work. They are the most requested days/shifts off in the year. Only those associates required to work on a premium paid holiday will be paid the premium for that day. Those associates who choose to work in lieu of taking a PTO day for a premium paid day will be paid their normal wages for that day worked.
14. Time off without pay. Regardless of status, associates must take a PTO day when taking time off on a holiday or any other scheduled workday when time off is granted. PTO must be used for any requested time off, and time off without pay will not be approved unless in conjunction with an approved leave of absence and there is no PTO available
