Associate Handbook
1. Minimum Pay Period Hours for Eligibility: Associates awarded positions regularly scheduled sixty (60) or more hours per pay period.
2. Executive Leader and Director of Nursing receive their awards in accordance with the schedule below:

Accrual Rate Per Pay Period
** For each subsequent year (see above schedule) up to 10, there will be 8 PTO hours added to the PTO accrual schedule for a maximum accrual schedule of 208 hours at 10 years and subsequent years.

* The Evening shift will be defined as the shift where the majority of the hours occur after 2pm and which extends beyond 7:30 p.m. An evening shift designation will be given to shifts that start before 10:00 a.m.
4. Separation of Employment: Terminating associates (those discharged or who resign and given appropriate written notice as well as the commitment to work through the notice period (two weeks for all staff associates and four weeks for supervisors and/or management associates) will be paid out all of the accrued and unused PTO as of their termination date. Associates who are terminated because of job abandonment or “no call/no show” are considered to have resigned without notice because they have abandoned their jobs and are not eligible for an out of PTO upon termination.
