(a) Login to the UTSW eIRB system: https://eresearch.swmed.edu/eIRB/
(b) Select “eCOI” from the super tab:
(c) Select “Create Statement”:
(d) Navigate the eIRB prompts pressing “continue” at the right-hand, bottom side of the screen as you complete each of the fields with accurate information reflecting your interests and potential COI.
(e) Once you have completed the form press “finish”.
(f) Addressanyfurtherreportingpertheinstitution’srequest.
2) The covered individual must disclose any newly acquired COI at the time they are acquired. Any new or changed COI must be reported within 60 days of acquiring it.
5.3 Review & Report
All submitted COI must be reviewed by the institution for their relatedness to the study and to the degree of impact for which a related COI may have on the integrity of the research. If it is determined the COI is a significant, financial COI, it must be submitted prior to spending or renewal of the awarded funds from the funding mechanism. Annual updates on the management of all COI must be reported. Please, see Table 5.3 below for an outline of the information required in the reporting of significant COI.

The name of the investigator with the FCOI.
The nature of the FCOI.
