              Discharge Medicines Review (DMR)
              When patients are admitted to hospital or
              another care setting, they may have their
              medicines changed, which on discharge
              if not communicated effectively to the
              patient’s GP, can result in problems with
              subsequent prescriptions.
              A DMR involves carrying out a two part
              structured review of a patient's medicines:

              Part one: Ensure medicines prescribed
              by the patient’s GP when discharged,
              match those medicines prescribed by the
              hospital (or other care setting). This must be
              completed within 4 weeks of the discharge.

              Part two: The pharmacist/technician and
              patient will, at the agreed time and by the
              agreed method, discuss the patient’s use of
              medicines since discharge.
