Seller Property Questionnaire (SPQ)
The Seller Property Questionnaire is a form to be completed by a Seller in an effort to give Buyer a more
comprehensive understanding of the property to be acquired. The form does not replace the Real Estate
Transfer Disclosure Statement but supplements it. This form is not mandated by statute or contract but
may serve as a helpful tool in a transaction. The four pages of questions can often prompt Seller to

warranty regarding the property's condition.
Seller Vacant Land Questionnaire (VLQ)
This form contains a list of questions asking about the Seller's knowledge of the property. Since sellers of

This form is used to document that a Buyer has been advised to obtain certain inspections but has

Market Conditions Advisory (MCA)
This form is used to document that a Broker discussed with a Buyer that market conditions fluctuate and

advises about the risks of making non-contingent offers or removing contingencies.
Megan's Law Data Base Disclosure (DBD)
Complies with requirement that certain printed information be part of residential real estate contract and
