               Statewide Buyer and Seller Advisory (SBSA)
               This document advises both Buyer and Seller of various factors that may affect the decision to buy,
               different types of inspections that can be obtained, and questions to ask about or research and

               information to be disclosed. Buyer is advised in each particular section to contact appropriate
               professionals for concerns. Designed to work in conjunction with local disclosure addenda. This form is

               not mandated by statute or contract.

               Water-Conserving Plumbing Fixtures and Carbon Monoxide Detector Notice (WCMD)
               Modifies existing Carbon Monoxide Detector Notice to add language about water conserving plumbing

               devices. Identifies the compliance and disclosure requirements of the law and State law standards to
               assess if plumbing fixtures are "non-compliant."


               Water Heater and Smoke Detector Statement of Compliance (WHSD)
               Seller uses this form to certify that water heaters and smoke detectors are properly installed in
               compliance with state and local laws.


               Wildfire Disaster Advisory (WDFA)
               Addresses major concerns and issues of buying or renting property in wildfire area. Form puts buyers and

               tenants on notice and provides contacts for additional information

               Homeowner’s Guide to Earthquake Safety Booklet (EQ Receipt)


               The licensee must give the transferor (Seller) the booklet The Homeowner’s Gide to Earthquake Safety
               and the transferor (Seller) must give the booklet to the transferee (Buyer).

               Residential Earthquake Hazards Report (EQ Report)


               Transfer of residential real property of 1-4 units, manufactured and mobile homes, of conventional light
               frame construction, and built prior to January 1, 1960.

               Known structural deficiencies must be disclosed by the transferor (Seller) to the transferee (Buyer) and

               the form in the booklet entitled Residential Earthquake Hazards Report must be used to make this
               disclosure.
