Licensed Group Home Setup For GRO, RTC HCS and

Foster Group Homes

* ( In order to open a Licensed Home you must attend the RCCL

class first to receive documents)
1. Present documents to Licensing Agency in a three ring binder

unless electronic submittal is required.

2. Application for license/fee
3. Copy of Articles of Incorporation or Articles of Organization

4. Copy of the Constitution and bylaws ( mostly for nonprofits)

5. Nonprofit Application

6. Three month or Six month startup budget with proof of
reserved funds ( varies depending on the state)

7. One year Projected Expenses Budget

8. List of Officers and governing body with contact information
• State the terms of service for each position of the Board

9. Operation Procedures that conforms to state regulations

including:
• Statement of Purpose/ Mission Statement

• Objective
• Scope of Services provided

• Specify targeted youth

10. Admission

11. Proof of education/ experience for program director and

licensed staff such as licensed social worker/ counselor or
registered nurse. This may include resumes, copy of bachelor's

degree and criminal background check forms for each employee
