P. 59

• Indications:
o Distal radius and ulna fractures
                          o Carpal fractures
                          o Post-surgical wrist stabilization
                   • Application Steps:

                          1. Apply stockinette and padding.
                          2. Wrap fiberglass or plaster evenly around the forearm and wrist.
                          3. Mold appropriately for wrist positioning.
                          4. Allow time to set before moving the limb.

               2. Long Arm Cast (LAC)

               Indications:

                   • Midshaft radius and ulna fractures
                   • Elbow dislocations or post-reduction stabilization
                   • Severe wrist fractures requiring elbow immobilization
                   • Post-surgical forearm stabilization

               Application Steps:

                   1. Prepare the Limb:
o Position the elbow in 90 degrees of flexion with the wrist neutral or slightly
                              extended.
o Ensure patient comfort and proper anatomical alignment.
                   2. Apply Stockinette and Padding:
o Extend stockinette above the elbow and below the wrist.
o Use additional padding over the olecranon, medial/lateral epicondyles, and
                              ulnar styloid.
                   3. Apply Casting Material:
o Wrap the cast from just below the metacarpals to the proximal humerus.
o Avoid excessive tightness over the antecubital fossa to allow elbow motion if
                              needed.
                   4. Molding and Setting:
o Mold along the distal radius and ulna to maintain fracture reduction.
o Ensure full set before moving the patient to avoid cast shifting.

                   • Indications:
o Midshaft radius and ulna fractures
                          o Elbow dislocations
                   • Application Steps:

                          1. Extend cast from the hand to just above the elbow.
                          2. Ensure elbow is at 90 degrees for proper immobilization.
                          3. Reinforce padding at the elbow to prevent pressure sores.
